That sounds like a port forwarding Issue in the router, have you opened the port 5001 in the router?.
Also, what are you using to connect to the Slingbox the IP address or is it the finder ID?

Honestly I hate to sound angry but I've been working this problem for almost two months now. Here is my set up.
I have a Slingbox SOLO connected to an Actiontec MI424-WR router provided by Verizon. I get Verizon FIOS broadband service. The slingbox is hooked up in New York...I'm currently living in London, UK and trying to connect remotely.
From the first time I tried to use the thing from here in London it streamed for 5 minutes at a time before freezing. I looked around on the Slingcommunity boards and found a thread for this exact problem, with this exact router and FIOS.....I tried all the listed "solutions" and none of them worked. Now for some reason I can't connect at all and I get the classic Slingbox error message about checking to see if the slingbox is turned on and not blocked by Firewall/NAT settings.
Based on the talks I've had with dozens of pissed off people on these boards...about 50% of all slingbox owners can't seem to get their slingboxes properly working. I've sat online for hours with Slingbox tech support.....my brother back in the US has done the same, and also spent time on the phone with Verizon.
Basically....with the time we've both put in trying to get my slingbox working for remote viewing we could have planned a deep space mission by now. And the most annoying part is that every single person at Slingbox tech support seems to know LESS about fixing this stuff than I do. And I don't know very much at all! Plus I've never recieved anything approaching an explanation as to WHY my 180 dollar shiny new slingbox is basically functioning as a paperweight.
Granted...everything electronic has problems but I can't believe the sheer volume of Slingbox owners who can't get their boxes working! Plus, the main attitude I've gotten from Slingbox Tech Support is...."hey we already got your money so f**k off" I really don't know what else to do....almost 2 months screwing around with this and no solution in sight...I DON'T EVEN KNOW WHAT THE PROBLEM IS!
OK I just want to say that about 2 hours after posting this I tried connecting again after not changing anything on either end. And amazingly it now works. And I've just watched most of tonight's Yankee game without any problems or freezing or anything.
Let this be a lesson...if your slingbox doesn't work...post an angry frustrated message.
In all seriousness....I have no idea why it started working.

Sling/OpenVPN is possible through the FAA government proxy server via TCP port443 using either amcproxy.faa.gov:8080 or awaproxy.faa.gov:8080...still working as of Sep08 (DOES NOT Work w/SlingPlayer2.0!)

In all seriousness....I have no idea why it started working.Slinging can be affected by your home network as well as the computer you are using. A faster processor, adaquate RAM, and whether your home network is in use by another computer can have an effect on slinging.

I know nothing about routers, but would this port setting that you refer to have anything at all do with wild swings in streaming speeds? Mine never really outright freezes, maybe once in a blue moon.
Also what is the difference if you use the finder ID to connect to your Sling vs. the IP address? Not sure I understand your question to the OP? TIA for the clarification and any help.

Man, I have to say...now that my box is working properly this is the greatest piece of electronics I've ever owned. It's been running consistantly at around 1700 kbps and I honestly can't tell the difference in picture from my home TV most of the time.
Plus I can access my DVR and all the on demand features of my home box...the only slight glitch is I've noticed that often once I start an on demand program or watch a saved dvr item....I can't pause it during play because after you pause it, it won't unpause. But I can live with that. (I assume its something with the electonic eye on my home box.
It was hard to get this thing working...but I have to say it was well worth it. I've been able to watch all my hometown NY sports that I was missing since I moved to London. For me just that was more than worth the money I paid for the slingbox.
